With our Diabetes Management Program, we seek to lower the barrier of entry and continuation of care for Associates living with diabetes by providing FREE diabetes management and care.
What Included?
- FREE 1-on-1 sessions with Health WoRx Clinical Staff (frequency determined on a case-by-case basis)
- FREE medical testing supplies (glucometer, CGM)
- FREE oral and injectable medication options
***The Diabetes Management Program is not intended to replace your current providers (PCP or endocrinologist). Health WoRx Clinical staff works with your current providers to offer additional support in managing your diabetes. For the full scope of program limitations, please contact Health WoRx.
